Before you begin

Every nomination package must include one item from each group below. Gather them first so you can finish in one sitting.

1. Nomination summaryA written narrative of up to three pages, or a video or audio narrative of up to five minutes.
2. Photo or logosA headshot of the nominee, or logos of the nominated community partnership.
3. Resume/CV or letterThe nominee’s resume or CV, or a letter of recommendation from the nominator.
