Resilient Kern works to serve all residents of the county by: - continuing to expand interdisciplinary care coordination among 100+ Kern County partner organizations - being selected by the first in the nation statewide ACEs Aware grant Initiative* to host Adverse Childhood Experiences (ACEs) provider training - promoting a new vital sign – one’s ACE score - through regular ACE screening services at local health clinics and managed care entities to greatly reduce levels of intergenerational Adverse Childhood Experiences (ACEs) transmission that can lead to poor lifelong health and wellness outcomes - leveraging state of the art closed loop referral technology resources - coordinating a public information and community outreach campaign (see ).
